促愈瘢痕消皮肤局部用药对动物毒性作用的实验分析

【摘要】 目的 探讨促愈瘢痕消(70%)促愈瘢痕消对动物皮肤的毒理作用。 方法 40只新西兰大白兔,随机分为3组。①组12只兔子随机分为A、B、C 3组,每组4只,分别为赋形剂组、促愈瘢痕消临床用药剂量组(44%)、促愈瘢痕消大剂量组(70%)。进行双侧背部脱毛,左侧刀片搔刮至均匀出血,右侧用药7 d观察药物经完整皮肤的急性毒性作用;左侧用药7 d观察药物经损伤皮肤急性毒性作用。②组4只兔子每只背部对称脱毛各2块,药物分组同上,左侧均涂赋形剂,右侧头侧脱毛区涂促愈瘢痕消(44%),右侧尾侧区涂促愈瘢痕消(70%)。观察用药7 d后对完整与损伤皮肤刺激性。③组24只兔子随机分为A、B、C 3组,每组8只,分别为赋形剂组、促愈瘢痕消(44%)组、阳性对照组(1%2,4-二硝基氯代苯)组,进行左侧背部脱毛,各组外用药物进行初次接触及激发试验,观察药物对动物皮肤的过敏反应。结果 促愈瘢痕消经皮肤对动物完整皮肤及损伤皮肤无急性毒性、刺激性;药物对完整皮肤亦无激发过敏反应。 结论 促愈瘢痕消短期应用对动物皮肤是安全的。 【关键词】 促愈瘢痕消;经皮肤急性毒性;皮肤刺激性;皮肤过敏反应 【Abstract】 Objective To study the acute toxicity, irritation and sensitivity effect of Cuyubanhenxiao to New Zealand big white rabbit’s nomal skin and impaired skin.Methods Fourty New Zealand big white rabbits were pidesed into 3 groups.①12 rabbits were randomized into 3 groups, each group is 4 rabbits. The medicine is Vaseline,Cuyubanhenxiao(44%),Cuyubanhenxiao(70%).They were separately applied to A,B,C 3 groups. Each rabbit with 50 cm2 was molted on both sides of backbone to take the acute skin toxicity test. The right sides every group were used medicine(A,B,C) at 7 days, the nomal skin acute toxic effect was observed. The left sides were scratched to even hemorrhage and spreaded medicine on every group rabbit. At 7 days, the damaged skin toxic effect was observed.②4 rabbits were molted on both sides of backbone of every rabbit and there were 2 molted areas on each side of every rabbit.The medicine group was completely same with the first group.The left side of every molted area was spreaded by Vaseline. The right molted areas of near to the head were spreaded by B and the oher areas were spreaded by C 7 days later, the nomal and damaged skin irritation effect was observed.③24 rabbits were randomized into A, B,and C 3 groups, the number of each group was 8. The medicine was Vaseline, Cuyubanhenxiao(44%), 1%2,4Dinitrochlorobenzol. Each rabbit with 50 cm2 was molted on the left sides of backbone to take the the initial contact and the stimulation test. The sensitivity effect of medicine was observed.Results There were no acute toxicity, irritation effect, when Cuyubanhenxiao was spreaded on the rabbits’ normal and damaged skin and no sensitivity effect phenomenon to rabbits’ nomal skin.Conclusion It’s safe to appl to the skin of animals for shortterm. 【Key words】 Cuyubanhenxiao,acute skin toxicity,skin irritation,skin sensitivity effect 创伤、烧伤、外科手术等常引起不可避免的创面,然而创面的晚期愈合常常导致瘢痕的增生。创面的晚期愈合增加了感染机会,瘢痕增生给患者美观甚至功能造成严重影响。目前,国内外用于促进创面愈合及治疗瘢痕的药物种类繁多, 但经临床使用后,疗效好者甚少。本课题组研制的中药制剂促愈瘢痕消,经动物实验证明能促进创面愈合、预防瘢痕增生且无明显的毒性反应。本文仅就其动物毒理实验进行论述。 1 材料与方法 1.1 试剂 实验用促愈瘢痕消由山东大学第二医院美容整形烧伤外科研制。此中药制剂主要成分由土元、水蛭、莪术、蜈蚣、全蝎和五倍子(均由山东大学齐鲁医院中药房提供),采用超细微粉化新工艺精制而成。实验药物分临床用药剂量(药物浓度为44%)及大剂量(药物浓度为70%)。 对照用赋形剂为凡士林软膏(山东大学第二医院药剂室提供)、硫化钠、2,4二硝基氯代苯(济南泰瑞达药化有限公司提供)。 1.2 动物 实验动物为新西兰大白兔,雌雄不限,体质量2.5~3.0 kg,由山东省农科院提供(合格证号:scxk齿20040013)。 1.3 实验方法 1.3.1 促愈瘢痕消经皮肤急性毒性试验:将12只新西兰大白兔随机分为3组,每组4只,A组:赋形剂组;B组:促愈瘢痕消临床用药剂量组(44%);C组:促愈瘢痕消大剂量组(70%),用8%的硫化钠给白兔右侧背部脱毛50 cm2,用清水洗掉化学试剂,立即给脱毛区涂药,每日涂药使药膏保持浓度1周,期间观察动物的活动、皮毛光泽、饮食、体重、死亡情况。随后对白兔左侧脱毛50 cm2,并用手术无菌刀片在脱毛区进行搔刮至均匀出血,立即在擦伤部位涂药,用药7 d,观察指标同上,并与对照组比较。 1.3.2 促愈瘢痕消的皮肤刺激性试验:将4只新西兰大白兔背部对称脱毛4块,每块50 cm2。A:赋形剂,B:促愈瘢痕消(临床用药剂量组44%),C:促愈瘢痕消大剂量(70%)。脱毛后清水清洗脱毛区,左侧均涂赋形剂(A),右侧头侧脱毛区涂B,右侧尾侧区涂C。涂药后24 h温水清洗药物及赋形剂,观察局部皮肤红斑和水肿反应。连续用药7 d后,清洗掉药物观察24 h、72 h后的皮肤反应。随后脱毛区用手术无菌刀片搔刮至均匀出血,立即涂药物及赋形剂,观察指标同上。
